Partial Differential Equations by Lawrence C. Evans. The syllabus of Math 673/AMSC 673 consists of the core material in Chapters 2-4 and of selected topics from Chapters 5 and 6. You can find a list with errata for the first, the second, and the third printing on the author's home page. (Here is the Review of the book in the MathSciNet of the AMS.)

Math 411 or equivalent.
